Quarterly Spotlight: Clinical Documentation Excellence

The Clinical Documentation Excellence (CDE) Circle of Excellence Awards recognize clinicians who demonstrate excellence in clinical documentation—supporting better outcomes and high-value care.
Each month, CDE educators nominate outstanding specialists, primary care physicians and APPs for this distinction.
Congratulations to our January, February and March 2026 honorees:

Alan Wilson, MD
Alan Wilson, MD, practices family medicine at Ochsner Health Center in Covington, Louisiana. He was nominated by CDE educator Lauren Brettner for his strong engagement with the CDE program and commitment to high-quality documentation.
Dr. Wilson’s performance reflects his dedication, with a nearly 97% HCC reconciliation rate and consistent, clear support for chronic conditions across chart reviews. He is an active participant in CDE meetings and demonstrates a clear understanding of how documentation contributes to patient care.
We appreciate Dr. Wilson’s ongoing commitment to documentation excellence and are proud to recognize him as a Circle of Excellence honoree.

Clayton Eiswirth, MD
Clayton Eiswirth, MD, is a pulmonologist at Ochsner Medical Complex in Metairie, Louisiana. He was nominated by CDE educator Brianna Savoie for his engagement in the CDE program and his strong performance during Specialty Provider Evaluation, Education, and Development (SPEED) sessions.
Dr. Eiswirth’s 97% encounter action rate reflects his consistent approach to accurately documenting and managing chronic conditions for complex patients. He is responsive, engaged and reliable in his participation in CDE meetings.
We recognize Dr. Eiswirth for his dedication to high-quality documentation and are pleased to honor him as a CDE Circle of Excellence recipient.

Krista Balius, NP
Krista Balius, FNP-C, practices at OLG Family Medicine – Cordoba Clinic. She was nominated by CDE educator Traci Dufrene for her engagement, consistency and commitment to improving documentation quality.
Since beginning the CDE Focus Program in May 2025, Krista has demonstrated measurable growth and responsiveness to feedback, increasing her encounter action rate from 50% to consistently above 90%. Her professionalism and positive approach support both accurate documentation and high-quality patient care.
