3 Clinicians Recognized for Documentation Excellence
Accurate, thorough clinical documentation plays a vital role in providers’ ability to deliver high-quality patient care. The Circle of Excellence award program recognizes physicians and advanced practice providers who demonstrate exceptional dedication to documentation accuracy, engagement in Clinical Documentation Excellence (CDE) initiatives and collaboration with care teams.
These clinicians help ensure that each patient’s medical story is clearly captured in the record, supporting coordinated care, quality reporting and appropriate resource utilization.

Saira Amjed, MD, Primary Care
Our October 2025 Circle of Excellence recipient, Dr. Saira Amjed, a primary care physician at Ochsner Health Center – Lake Terrace, was nominated by her CDE nurse, Tiffany Dufour. Known for her warm and energetic approach, Dr. Amjed consistently demonstrates strong engagement during CDE education sessions. She actively participates by following along in Epic, identifying opportunities for improvement and collaborating with her CDE nurse. Her 92% reconciliation rate reflects her commitment to continuous improvement and excellence in clinical documentation.
Jamie Huddleston, MD, Neurology
November 2025 Circle of Excellence recipient Dr. Jamie Huddleston, a senior physician specializing in neurology at Ochsner Specialty Health Center – Raceland, was nominated by her CDE nurse, Lauren Hickey. Dr. Huddleston brings positivity, professionalism, and passion to everything she does. With a 97% encounter action rate and attention to more than 100 HCC alerts in 2025, she consistently sets a high standard for documentation excellence and patient-centered care.
Colleen Knoop, DNP – Advanced Practice Provider
December CDE winner Colleen Knoop, DNP, is a nurse practitioner in the Neurology department at Ochsner Medical Center – New Orleans, specializing in movement and memory disorders. Nominated by CDE nurse Lauren Hickey, she is recognized for her engagement during meetings and her strong commitment to patient care. With an impressive 98% encounter action rate in 2025, her documentation consistently provides clear support for coded diagnoses. Colleen is also known as a supportive collaborator who fosters a positive environment among colleagues.
