From Complex Needs to Renewed Health

When a 68-year-old male with complex chronic conditions entered Ochsner’s Outpatient Care Management (OPCM) program, his outlook was challenging. With uncontrolled Type 2 diabetes (A1C 9.8), CKD3b, and heart failure (EF 20%) requiring a LifeVest, he also faced significant barriers—financial instability, limited access to medications and a lack of understanding of his conditions.

“I didn’t really know how to manage any of it,” he shared. “It felt overwhelming.”

Through the OPCM program, a dedicated nurse case manager provided intensive education, coordinated medication adjustments with providers, and addressed financial barriers to care. Over time, the patient became increasingly engaged—adopting healthier habits, monitoring his health and actively participating in his care.

“The support I got made all the difference,” he said. “Someone finally took the time to help me understand and stay on track.”

The results were transformative:

  • A1C improved from 9.8 to 6.3
  • Ejection fraction improved from 20% to 55%
  • LifeVest discontinued, and access to Entresto secured

Today, he remains engaged and optimistic about his health.

“I feel like I have my life back,” he said. “I want other patients to know this kind of change is possible.”

The patient is now thriving and eager to share his experience. His story highlights how patient education, care coordination, and addressing social determinants can dramatically improve outcomes—even in the highest-risk patients.
